The full guide
Who can use Spain’s public health system?
| Your situation | How you’re covered |
|---|---|
| Employee or autónomo | Through your social security contributions |
| UK, EU or Swiss state pensioner | By registering your S1 form with Spanish social security |
| Spouse, partner or child of a covered person | As a registered beneficiary |
| Non-working resident | Private insurance, or the convenio especial after a year of residence |
People on the non-lucrative visa usually rely on private insurance, because they don’t work or contribute. See health insurance for a Spanish visa.
Step 1: get your right recognised
Before a health centre can register you, social security (the INSS) must confirm your entitlement. You can request this online through the Tu Seguridad Social portal with Cl@ve or a digital certificate, or at a social security office (CAISS) with an appointment. Employees are often recognised automatically once their employer registers them.
Step 2: register at your health centre
| Document | Notes |
|---|---|
| TIE, EU registration certificate or passport | Original and a copy |
| Padrón certificate | Recent, showing your address |
| Social security number | See getting a social security number |
| INSS recognition document | The confirmation of your right to healthcare |
The centre assigns you a GP and requests your regional health card. Each region issues its own card: in Catalonia it’s the TSI from CatSalut, in Valencia the SIP card, and so on. If you move region, you register again in your new region.
The convenio especial: paying in
If you’re not entitled through work, a pension or family, you can join the public system through the convenio especial once you’ve lived in Spain for at least a year and are registered on the padrón. The state fee is €60 a month under 65 and €157 a month from 65. You apply through your regional health service, and some regions add their own conditions.
Common mistakes
Going straight to the health centre. Get your INSS recognition first or you may be turned away.
Not registering dependants. Spouses and children need to be added as beneficiaries.
Forgetting the European card. Once covered, request the free European Health Insurance Card for trips in the EU.
Frequently asked questions
Can I see a doctor before my card arrives?
Yes. Use the registration receipt or your health record number. Emergency care is always available.
Does public healthcare cover dentists?
Only basic dental care such as extractions, and more for children. Most adults pay for dentistry privately. See dental care in Spain.
Can I keep private insurance too?
Yes. Many residents keep both for shorter waits for specialists.
